Youth Sports Motivation Questionnaire
(To Be Completed By Parents)

The following is a list of reasons that parents often give for encouraging their children to participate in organized sports programs. Please look at each reason and decide whether it is why you believe your child should be involved in his or her sport. If it is a reason that you encourage your child to participate, place a mark in the box under “I agree.” If it is not one of the reasons you encourage your child to play his or her sport, place a mark in the box under “I do not agree.” If you are not sure if it is a reason you encourage your child to play his or her sport, place a mark in the box under “I’m not sure.”
