Youth Sports Motivation Questionnaire
(To Be Completed By The Youth Athlete)
The following is a list of reasons that young people have given in answer to the question, Why do you play sports?
Please look at each reason and decide how much you agree with it. If a reason is one for which you play your sport, you
should place a mark in the box under “I agree.” If it is not one of the reasons you play your sport, you should place a mark
in the box under “I do not agree.” If you are not sure if it is a reason you play your sport, you should place a mark in the
box under “I’m not sure.”
